The Community Alternatives Program for Disabled Adults (CAP/DA) is a Medicaid waivered program that provides home & community based services to Medicaid recipients 18 years & older who would otherwise require care in a Nursing Facility. CAP/DA offers an alternative to institutional placement.
We provide quality care in the home for those needing assistance with:
- In-Home Aide Services: Assists with personal care (bathing & dressing), light housekeeping tasks, & meal preparation
- Case Management
- Skilled Nursing Services
- Physical, Occupational, and/or Speech Therapy
- Respite Care
- Adult Day Health Services
- Telephone Alert
- Oral Nutrients
- Mobility Aids
- Prescription Medications will be covered by Medicaid
CAP/DA Services are available to individuals at least 18 years of age who are certified ICF or SNF Nursing Home qualified by a physician & meet Medicaid eligibility guidelines.
The Community Alternatives Program for Children (CAP/C) is a special Medicaid community care program that serves a limited number of medically high risk children (under 18 years of age) who would face institutionalization in a nursing facility or hospital by providing:
- Screening/Assessment
- Nursing Services
- CaseManagement
- Board Certified Nurse Aide Services
- Speech, Physical & Occupational Therapy
- Durable Medical Equipment
- Medical Supplies
CAP/C Services are available to individuals less than 18 years of age who are certified ICF or SNF Nursing Home qualified by a physician & meet Medicaid eligibility guidelines.
The CAP/Choice program allows older and disabled adults the opportunity to direct and have increased control over the services and supports that are arranged to meet their needs.
